At checkout (or in your order notes), include:

• Name (exact spelling, capitalization)
• Number (ex: 21)

Double-check spelling—your file will be created exactly as submitted.

For best results, print on heavyweight matte paper or cardstock and select “fit to size” (avoid “stretch”).
